The Zombies mode of the Call of Duty series first appeared 18 years ago in Call of Duty: World at War and remains popular among many players. Spencer Sharp, the solo developer of Milo, described the project as a "love letter" to the classic Call of Duty Zombies mode. The game features WWII-inspired maps where players fend off waves of zombies approaching from different directions.

The demo and trailer showcase the core gameplay: killing zombies earns points and tickets, which can be used to unlock weapons from walls or purchase upgrades from vending machines. Side challenges appear every few rounds, such as eliminating zombies in a specific order, offering extra rewards upon completion.

Milo also introduces its own unique mechanics. A swarm of rats follows the player, devouring zombie corpses and dropping rewards; players can also play a basketball minigame between rounds to earn points for unlocking perks. The Early Access version plans to include over 12 WWII-era weapons, including the M1 Garand rifle.

The game also plans to add split-screen mode, though this feature will not be available at the Early Access launch. Developers are working on two new maps, "Oil Rig" and "Aqaba," along with new zombie types; the rat mechanic is also set to expand, with cosmetic options being added. Additionally, a new device is planned to grant effects such as "One Shot One Kill" and "Infinite Ammo."
