The ending of the 2026 remake of Resident Evil has been revealed: Brian undergoes complete mutation, Umbrella Corporation's antidote plan fails, and the Raccoon City outbreak spirals out of control. There are no post-credits scenes or easter eggs after the film enters the credits. The following content contains full ending spoilers.
The film centers on Brian, a medical courier. He is tasked with delivering an important item to Raccoon City General Hospital, unaware that the city has become the epicenter of a T-Virus outbreak. The infected transform into zombie-like monsters with tentacles, and the stabilizer he is transporting is the key material needed to complete the antidote.
Brian was bitten by an infected child along the way, after which he traversed farmhouses, alleys, and sewers as his condition deteriorated. He encountered infected individuals, swarms of mutated rats, and a monster resembling a "Rat King," finally reaching the isolated laboratory atop the hospital and handing over the stabilizer to the surviving Umbrella Corporation scientists.
The laboratory team promptly used the stabilizer to create a T-Virus antidote, but Brian's mutation accelerated at the same time. He gradually lost control of his body; extra limbs began attacking the lab personnel, and a massive maw that appeared on his abdomen swallowed several scientists. Although Brian repeatedly begged them to inject him with the antidote, no one could get close enough.
Amidst the chaos, syringes containing the antidote slipped from hands multiple times in the laboratory. The last scientist abandoned the injection attempt and tried to flee, but Brian knocked him out of a window. Both fell onto an skybridge connecting the hospital buildings. By this point, Brian had completely lost his humanity, his mutation worsening further, and he killed the scientist.
The syringe containing the antidote then falls into the snow below. After the National Guard helicopter arrives and discovers the fully mutated Brian, the camera zooms into his eyes and infected blood, before cutting directly to the end credits. The film ends here, with no additional scenes during or after the credits.
