The game is set in a world ten years after a zombie outbreak. The protagonist awakens from a six-month coma and must investigate their past while exploring ruins, forests, farms, and military facilities overgrown with vegetation. Players must not only fight off the infected but also contend with hostile human factions, all while maintaining the operation of a safe zone.

In addition to exploration and combat, the game includes survival elements such as resource gathering, crafting, base building, farming, hunting, and fishing. Weapons can be equipped with attachments like scopes, silencers, and extended magazines, and players can drive vehicles to traverse the map. The number of players supported in online co-op mode depends on the host's computer processing power.

Players can switch between first-person and third-person perspectives at any time, with weather and day-night cycles affecting visibility, combat, and missions. Set across three islands, the game features over 30 main quests and more than 1,000 voice lines. Player choices, mission outcomes, rescued survivors, weather, and time can all influence dialogue, character reactions, and quest endings.

Bases can be equipped with appliances powered by generators, with energy production dynamically changing based on environmental conditions: solar panels require sunlight, while wind turbines depend on wind speed. The game is planned for an Early Access release, during which the development team will incorporate community feedback to expand the story, locations, enemies, items, and survival systems. The developers stated that the game has already surpassed 185,000 wishlists.
